Benefits

At ZANTAZ, we believe our employees are our strongest asset, and we offer a comprehensive and competitive benefits program designed to protect the health and welfare of our employees and their loved ones. We continually examine the value of our benefit plans to be sure we are keeping pace with market trends and innovative approaches to health care and the financial security of our staff.

ZANTAZ offers employees a choice of benefits and the freedom to customize coverage each year to fit individual or family needs. We offer the following benefit programs to all regular U.S. employees who work 20 or more hours per week. Coverage is also available for domestic partners, spouses and eligible dependents.

Medical

Employees can choose from HMO type plans, or Preferred Provider options offered by leading health care organizations, all with comprehensive networks. Plan options vary by location.

Dental

The dental plan offers traditional coverage with a higher benefit network option.

Flexible Spending Accounts

Employees can reduce taxable income and extend purchasing power for health care and dependent care expenses by enrolling in one or both of the following pretax spending accounts:

Health Care Spending Account: Employees can use the pretax dollars they put into this account to pay for many of the health care expenses not covered by medical, dental and vision plans, including deductibles and co-payments.

Dependent Care Spending Account: Employees can use the pretax funds in this account to pay for eligible dependent care expenses.

Disability

To help protect employees' income during times when they can't work, ZANTAZ provides short-term disability insurance (at 60% of base salary). ZANTAZ also offers voluntary long-term disability insurance that protects employees' income (at 60% of base salary).

Life Insurance and AD&D (Accidental Death and Dismemberment)

ZANTAZ' competitive coverage provides a base amount of life insurance equal to one times base salary. In addition, employees can "buy up" for additional levels of coverage up to $500,000 or 5X their annual salary. They may also purchase life insurance for a spouse, child or domestic partner at group rates.

401(k) Plan

ZANTAZ' 401(k) plan lets employees accumulate savings on a tax-deferred basis. Through payroll deductions, employees can direct up to 60% of pretax earnings (up to an IRS annual maximum) into the Fidelity savings plan. ZANTAZ also matches a portion of the employee's contribution amount. If employees are age 50 and older, they may also contribute additional catch-up contributions, subject to IRS maximums. Employees may enroll, increase, decrease, change investment funds, transfer funds or discontinue contributions monthly.

Paid Time Off (PTO)

ZANTAZ offers a generous Paid Time Off (PTO) plan. PTO is an all purpose time-off policy for employees to use for vacation, illness and personal business. It combines traditional vacation and sick leave plans into one flexible, paid time-off policy. Regular full-time employees will accrue 4 weeks (160 hours) of paid time off per year in their first 5 consecutive years of employment. After 5 consecutive years of employment, regular full-time employees will accrue 5 weeks (200 hours) of paid time off per year.

Holidays

ZANTAZ observes 10 paid holidays each year.

Employee Assistance Program (EAP)

The ZANTAZ Employee Assistance Program offers confidential counseling and referrals for personal, family, financial, health or work-related problems and legal advice to employees and their enrolled family members.

Referral Programs

This free resource and referral service helps with child care, elder care, pet care, adoption assistance and school selection from grade school through college.

Eligibility and participation in the plans described above are subject to the terms of the actual plan document and ZANTAZ' policies and procedures.
